You r giving ur landlord notice that u do not intend to renew the lease? Just write a simple note to that effect, be sure to date & sign it & send it via certified mail with return receipt requested. You get the certified mail form at the post office, so just stop there to mail the letter instead of dropping it in a mail box.

If u r a landlord & r notifying the tenant that u will not be renewing their lease, the particulars of the letter should match the provisions in the underlying lease.
